Summary

Tranexamic acid is a synthetic derivative of the amino acid lysine that inhibits plasminogen activation to plasmin. Topically, it is thought to reduce melanin synthesis by inhibiting plasminogen activator in keratinocytes, which in turn reduces the activity of tyrosinase. This mechanism helps to diminish hyperpigmentation and erythema, improving skin tone and reducing discoloration often associated with aging or sun damage.

Purpose

Reduces hyperpigmentation and redness, improving skin tone and appearance.

Card Summary

Impact

Score:
4.5
Justification:
Effective for targeted skin pigmentation and redness, not systemic healthspan.

Evidence

Grade:
A
Score:
8.5
Justification:
Robust human RCTs and meta-analyses support efficacy for melasma.

Safety

Score:
3.5
Justification:
Generally well-tolerated topically, minimal systemic absorption, mild irritation.

Flags

  • Mild skin irritation, dryness, or redness can occur, especially initially.
  • Effectiveness for hyperpigmentation may require consistent, long-term use.
  • Systemic absorption is minimal but theoretical risk of hypercoagulability for large-area application.
  • Should not be used on broken or irritated skin.

Verdict

Effective for targeted skin hyperpigmentation, improving cosmetic appearance.
